This document authorizes minors to travel alone or with one of the minor’s parents or legal tutor. This document requires:

• A valid document that identifies the father, mother, or the legal guardian authorizing the minor to travel

• A valid document that shows the maternity, paternity, or guardianship of the minor

• Full name of the minor

• Birth date of the minor

• If the minor is traveling alone, the authorization of both parents, or legal guardian is needed

• The minor does not need to appear at the Consulate for this service

• The cost of the document is US$ 7
