Visa applications form incomplete may be rejected; and all the
applications are subject to verification.

Original passport and a color photocopy (only the
page with photo).
(The passport must be valid at least for the term of the visa 18
months minimum).

Health Certificate that prove you are free of
contagious diseases, issued within 30 days previous you apply for
the visa. This Certificate must be issued by applicant's School
Health Service or private Physician (Medical Doctor), on issuer’s
letterhead.
We do not have a Health Certificate Form.

F.B.I. Report ((304) 625-2000 (202) 324-3000). No
more than one month previous to the date you apply for the visa.
Children under 15 years old not required.

Proof of financial status. Letter of economic
independence/support signed by the parents or a Statement from the
Educational Sponsor ( AFS, University, etc.).

Certificate of enrollment issued by the School or
University in Chile. This is a proof of enrollment in a Chilean
School or University, indicating that the student has been accepted.

Certificate of enrollment issued by your current
School, University or Student Exchange Program.

If the student is a minor (under 18 years old)
unaccompanied by parents, a written Authorization Letter (in
Spanish) to leave Chile signed from both parents is required.
This Authorization Letter, must be certified by a Notary Public and
by the County Clerk or by the Secretary of the State (Who
certificated the Notary Public’s signature).
You have to legalize this letter at this Consulate. The legalization
fee is $12.

Visas fee are: for American citizens US$160, Mexican
citizens $ 130, Japanese citizens $50, British citizens $ 300.

CONSULATE. WHEN THE CONSULATE INFORME YOU, THAT YOUR VISA IS READY,
YOU CAN COME TO PICK UP YOUR VISA.
