Valid Passport (Valid at least for the term of the visa)

Health Certificate issued by medical doctor stating that applicant is
in good health and has no contagious diseases.

It is necessary to present a notarized contract of employment signed
by both parties, employer and employee or his representative in Chile
along with the aforementioned documents.

Police certificate issued by your Local, State or Federal authorities
stating that the applicant has no records.

1 recent passport photograph (2" x 2 ")

This visa is valid for a maximum of two years and is renewable in
Chile.

If contract upon which this visa was granted is terminated or if
applicant change employment, his/her visa will expire automatically.

Before issuing a contract visa, authorization from Chile is required.
